Researchers identify possible imaging method to stratify breast cancer ... - Science Centric

Scientists from the Kimmel Cancer Centre at Jefferson have discovered a possible way for malignant breast tumours to be identified, without the need for a biopsy. The findings were published online ahead of print in the Journal of Nuclear Medicine.

Current imaging modalities miss up to 30% of breast cancers and cannot distinguish malignant tumours from benign tumours, thus requiring invasive biopsies. Approximately 5.6 million biopsies performed in the United States find only benign lesions. These biopsies cause substantial stress for the patients and have significantly high costs.

'The challenge has been to develop an imaging agent that will target a specific, fingerprint biomarker that visualises malignant breast lesions early and reliably,' said Mathew Thakur, Ph.D., professor of Radiology at Jefferson Medical College of Thomas Jefferson University and director of Radiopharmaceutical Research and Nuclear Medicine Research.

Dr Thakur and colleagues studied an agent called 64Cu-TP3805, which is used to evaluate tumours via PET imaging. 64Cu-TP3805 detects breast cancer by finding a biomarker called VPAC1, which is overexpressed as the tumour develops.

The researchers compared the images using that agent with images using the 'gold standard' imaging agent, 18F-FDG. They used MMTVneu mice, which are mice that develop breast tumours spontaneously, like humans. The mice first received a PET scan using the 18F-FDG. Then they received a CT scan, and then they received another PET scan using 64Cu-TP3805.

Ten tumours were detected on the mice. Four tumours were detected using both 18F-FDG and 64Cu-TP3805, and four additional tumours were found with 64Cu-TP3805 only. All eight of these tumours overexpressed the VPAC1 oncogene on tumour cells and were malignant by histology. The remaining two tumours were benign and were detected only with 18F-FDG. They did not express the VPAC1 oncogene, and thus were not detected by the 64Cu-TP3805.

'If this ability of 64Cu-TP3805 holds up in humans, then in the future, PET scans with 64Cu-TP3805 will significantly contribute to the management of breast cancer,' Dr Thakur said.

A new statement on breast cancer by the American College of Surgeons : (ACS) underscores the value of an advanced approach to post-surgical radiation therapy provided by the Breast Clinic of Memphis : .

The ACS document describes the advantages of accelerated partial breast irradiation such as SAVI™ : radiation therapy provided at

the Breast Clinic of Memphis.

The consensus statement says accelerated partial breast irradiation "is an approach that may allow more patients to undergo breast-conserving therapy more quickly, at lower cost, and with less risk of long-term complications." Among the techniques it lists for delivering APBI is brachytherapy : with "open single-entry devices with multiple lumens," a category that includes the SAVI applicator.

"The design of SAVI is quite ingenious in the way it resolves problematic issues presented by other breast brachytherapy methods," said Michael Berry, M.D., a surgeon at the Breast Clinic of Memphis who is also a course instructor for ACS. "It's easy to place and comfortable for patients. The device's expandable array of multiple catheters allows us to customize the radiation dose to the patient's needs and anatomy.

This creates a safer procedure : for women with certain body types or tumor locations."


Working with Dr. Berry to provide SAVI is radiation oncologist Michael Farmer, M.D., of the Memphis Professional Radiological Corporation (MPRC).

"This device is more versatile than balloon brachytherapy because unlike the balloon, it can be used with small-breasted women and women whose lumpectomy cavities are close to the skin," Dr. Farmer said. "You can lower the dose to healthy tissue such as the heart, ribs, and lungs and then concentrate the dose on the area where it's really needed. This advantage makes the benefits of breast brachytherapy available to significantly more women."

SAVI is one of the newest methods for providing APBI, a shortened course of high-dose radiation therapy for early-stage breast cancer patients following lumpectomy surgery. The treatment is completed in just five days — compared to the six weeks of treatment, five days a week, required for traditional, external-beam radiation.

The device's unique multi-catheter design : allows physicians to target radiation to the area that needs it most, minimizing exposure to healthy tissue. Precisely sculpted radiation is delivered from within the breast and targets the area where the cancer is most likely to recur.

Clinical studies : show that SAVI provides greater flexibility and fewer complications compared to other forms of breast brachytherapy.

Breast-cancer results of women who died will be revealed, health ... - Global TV.com

QUEBEC - Health Minister Yves Bolduc said Wednesday that the families of the five women with breast cancer who died, out of 2,856 whose results were retested, would be informed of the new results.

A ministerial directive, from the provincial health department, advised caregivers not to pass on the retest results to the families of patients who were deceased.

Bolduc said, in unveiling the results of an $800,000 verification of results by Quebec cancer labs, that the directive dated back to last August.

"We want to be transparent," Bolduc told reporters.

Asked if he feared lawsuits, Bolduc said he will wait for the outcome of court consideration of a class-action suit arising from concern over variations in testing results.

But he said the retests show Quebec has "the best laboratories in the world."

The review of tests done in Quebec's 58 pathology labs, on tissue samples of 2,856 breast cancer patients by PhenoPath Laboratories in Seattle indicated that cancer treatment for 39 Quebec women should change.

For 37 of the patients, hormone treatments are indicated. For two patients, treatment with Hercepton is advised.

The verification was ordered after Dr. Louis Gaboury, who heads the Quebec association of pathologists, presented a study last May, finding a rate of error between 15 per cent and 30 per cent in Quebec lab tests.

Only 15 tissue samples were tested.

Bolduc said he ordered the retests to reassure cancer patients.

André Robidoux, the oncology surgeon at Montreal's CHUM teaching hospital who headed a committee of experts named by Bolduc, said the hormone treatments, which can last for five years, offer a 50-per-cent chance there will not be a recurrence of breast cancer and a 15-per-cent better survival rate.

Robidoux said the low rate of error in the Quebec tests was "remarkable," adding that a 100-per-cent testing result is impossible and the results of the original Quebec tests are much better than tests elsewhere in the world.

"The first priority was patient care," he said.

Robidoux said he does not have enough information to determine whether faulty test results had anything to do with the five deaths among those retested.

"I can't answer that," he said.

"Every 12 minutes a woman dies in North America from breast cancer," Robidoux added. In Quebec, 3.9 per cent of breast-cancer patients die of the disease.

Robidoux said that 25 per cent of women who die in Canada each year die of breast cancer.

"It's a terrible disease."
